2013-14 Sports Eligibility Information

Students who plan on participating in a fall sport for the 2013-14 school year must complete a Sports Eligibility packet and a physical exam. The Sports Eligibility packet with physical exam forms can be picked up in the Main Office or from the Athletic Director’s office in Billiart Hall anytime after Monday, June 3. These forms are not on the website. The only form that can be found on the website is the physical exam form, which will become available on June 3. The Sports Eligibility packet and the completed physical exam form must be turned in to the Athletic Director before a student can practice or try-out for a sport.
