Athletic Resources

Students who plan on participating in a sport for the 2022-23 school year must complete the following requirements:
 

1. Access https://www.homecampus.com/login and fill out the online eligibility packet. This needs to be updated every school year.
    
2. New athletic participants will need to take the online courses – Sportsmanship and Social Media, Concussion in Sports (athletes only). Click on NFHSlearn.com. You will need to upload the certificates to your athletic clearance account to verify completion.
    
3. Download, complete and upload the following forms to https://www.homecampus.com/login

             Preparticipation Physical Evaluation form 
             Preparticipation Medical Eligibility form 
             Physical Examination form  - Upload a completed Physical form by a physician     
              within the past 12 calendar month. The back of the form needs to be stamped by your physicians                  office.

4. Baseline Concussion Test – The Athletic Director will email a link and instructions for taking the test. 
    
5. A Participation fee of $150 will be charged to your FACTS account.

All required paperwork must be uploaded to www.athleticclearance.com before for the athlete can participate on her team.

 

Diocese of Monterey Requirements
We welcome and encourage Notre Dame families to get involved by volunteering. However for the safety of the students, prior to volunteering for any event or activity in which there is direct contact with students (eg. transporting students to and from an athletic game or chaperoning a field trip) the volunteer is required to:

1. Be fingerprinted and cleared by the Diocese of Monterey. Download the Live Scan form for Volunteers:  

2. Provide certification of completion for the online course “Protecting God’s Children® Online Awareness Session 3.0”. To take the course please click here on  VIRTUS. 

3. Complete a Diocese of Monterey Driver Information Form. Please sign up as a volunteer then take the Defensive Drive 1 training program.
